In 2007, the WHO recommended voluntary medical male circumcision as an HIV prevention strategy across 14 African nations. Three randomized controlled trials. A reported 50–60% reduction in HIV acquisition risk. Billions in international donor funding. Millions of procedures performed.

The press covered the announcement. Nearly two decades later, the harder questions remain:

• Were the trials conclusive enough to justify continent-wide surgical intervention?
• Why did the recommendation exclude most of the world?
• Were informed consent protocols adequate across 14 culturally diverse nations?
• If circumcision reduced risk by 50–60%, why did health agencies insist on continued condom use?

Both sides have merit. Both deserve scrutiny.
